Individual Plan

$499/yr

  • Two prophylaxis cleanings
  • Two periodic exams
  • All prescribed bitewing and periapical x-rays
  • Two fluoride treatments
  • Enjoy a 15% discount on most all other dental services for a full year!
  • Patients who have had scaling and root planing or that are seen for periodontal maintenance cleanings will receive two credits of the fee for a prophylaxis cleaning toward two of their periodontal maintenance cleanings. Any additional cleanings will receive the 15% discount.
